Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person living with dementia with complex needs. Based within multi-disciplinary/agency community teams and working alongside nurses, therapists, primary care, social care and third sector providers, amongst others, you will also benefit from being a part of a Clinical Network of Admiral Nurses working across Wales and beyond.

We are looking for an exceptionally skilled, Registered Nurse/s (adult/mental health/learning disability) with a passion for, and demonstrable track record of, providing person-centred and relationship-centred dementia care with a strong sense of advocacy and equalities for this client group. Combining direct work with families with supporting the best practice of health and social care professionals, you will call upon your impressive knowledge of bio-psychosocial interventions to improve the experience and quality of life of families affected by dementia.

This is a full-time post however job share will be considered., The post holder/s will need to have the ability to work in a variety of settings, across a significantly rural geography. The role will include managing an individual caseload of carers and families with complex needs, running groups and providing education/ training.

Dementia UK will support you to maintain and strengthen your clinical development as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities. You'll become a welcome part of a network of Admiral Nurses and will have the opportunity to access relevant post-graduate training and develop professionally, informed by the Admiral Nurse Competency Framework.

The Admiral Nurse Team is one of a range of initiatives being established across the counties of Carmarthenshire, Ceredigion and Pembrokeshire. There will be seven (full time) Admiral Nurse posts situated one within each 7 Locality Clusters where health and social care teams have maximized integration and co-located team bases, of which the Admiral Nurses will be an integral part.

This is an exciting collaboration between Dementia UK, Hywel Dda University Health Board and the respective Local Authorities of Carmarthenshire, Ceredigion and Pembrokeshire and is a cornerstone of the Dementia Wellbeing Pathway 'wrap-around' support for families affected by dementia., Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres
