Full Job Description

An exciting opportunity has arisen for an experienced Nurse to work as an Associate Specialist Palliative Care Nurse (AfC Band 6), based in our friendly, supportive team in both out South and Central PLACEs.

The Community Specialist Palliative Care Nursing PLACE teams are based at locations across Norwich, North, South and West Norfolk providing expert clinical palliative and end of life care for patients in their own homes and care facilities.

The service also provides a palliative care advice line which operates from a coordination centre at the new purpose-built Priscilla Bacon Lodge, in Colney Norwich.

The teams work as part of the wider Specialist Palliative Care Service which includes a consultant-led specialist inpatient unit, a Psychological Therapies team and a Day Therapies service.

We currently have opportunity for a part time or full time post., To deliver evidence-based clinical care for patients with life threatening illness who have complex specialist palliative care needs, from the point of diagnosis onwards under the supervision of the community Specialist Palliative Care Nurses.

To manage their own supervised clinical caseload, supporting patients and their families within their homes and in outpatient settings. The post-holder will link closely with community nursing teams, Hospice at Home/Supported Care, primary care, acute palliative care teams and the rest of services within specialist palliative care. The primary aim is to ensure continuity of care through all healthcare settings.

To undertake shifts as part of a clinical team operating the palliative care advice line service for professionals and patients/carers, providing clinical support and advice to any service users accessing this service.

To contribute to and be involved with audit, education/teaching and research.
