Carers Case Worker - East Dorset Locality Team

An exciting opportunity has arisen for a part time Carers Case Worker position (18.5 hrs a week) in the Adult Carers Team, covering the East Dorset Locality.

You will be based in a well-established, enthusiastic team, receive regular support from your line manager and have the support of an experienced team who work across Dorset. The post holder will hold a caseload and undertake strength-based assessments (recording these on our computerised client database), working with carers both within the hospital setting and in the community. The role includes providing emotional support and guidance to carers and their families, working collaboratively and creatively with other professionals whilst working flexibly.

The Carers Case Worker supports new and existing carers identified as being under stress. Support might include managing loss, supporting through difficult decisions, identifying, and voicing discharge options, funding options, mediating, and managing conflict, confidence building, offering information on carers support, facilitating links with community support and assessments all of which contribute to improving the wellbeing of the carer. Experience, skills, and knowledge of carers support is essential along with sound computer literacy., Interviews will be conducted in person in Ferndown Locality Office on Tuesday 21st and Wednesday 22nd May.

This is a part time role at 18.5hours per week.
