Full Job Description

The main duties of the role will be:
+ Medicines reconciliation and review on admission.
+ Providing support and information to patients and carers to assist with the management of their medicines.
+ Optimising medicines, taking a holistic view of both mental and physical healthcare needs.
+ Working in off-site community teams co-ordinating the process of medicines optimisation.
+ Working in Clozapine clinics providing medicines management support, patient reviews and performing venepuncture.
+ Providing physical health and wellbeing advice to patients and staff.
+ Discharge planning, liaison with patients, community teams and pharmacies.
Operational support to the dispensing services at the main hospital sites at St Georges Park and St Nicholas hospital by performing regular accuracy checking slots in the department and out on the wards.

We provide pharmacy and medicines optimisation services to all of our mental health, learning disability and neuro-rehabilitation inpatient areas, as well as to community teams, Clozapine clinics and crisis teams.
