Full Job Description

Communication & Working RelationshipsRecognising the patient as an individual and helping them to meet his or her social, emotional, health and intellectual needs. Develop relationships with patients, carers and colleagues which allow effective communication.Maintain open communication with patients, carers and staff.Receive visitors.Record information on charts and records.Input data on patients' electronic recordsTake, record and report telephone discussions and requests.Planning & OrganisationTake part in ward/ team meetings.Will be involved in the delivery and coordination of ward activitiesAnalyticsProvide and receive routine information to be able to assist in the undertaking of this role.Attend all mandated training as expected by the Trust.Attend any additional training as required for your role.Have an understanding of the statutory requirements of the Mental Health and Mental Capacity ActsResponsibility for Patient/Client Care, Treatment & TherapyRecognising the individuality
of the patient and help to meet his or her social, emotional and intellectual needs.Will be involved in the delivery and coordination of ward activitiesEnsuring that basic physical needs are met, such as adequate diet, hygiene, warmth, comfort and safety.Assisting in planned treatments in accordance with the multi-disciplinary team approach.Encouraging and assisting patients to participate in group and individual therapeutic activities.Accompanying patients on therapeutic activities both on and off the unit.Assisting in the promotion of comfort and hygieneMaintaining patient privacy, by use of appropriate available aids, e.g. screens, curtains etc.Assisting in the running of groups and activities.Assist with management of challenging behaviour (verbal and physical aggression) under supervision from trained member of staff. This may require the restraint of patients once suitably trained.Work as co-key worker and participate in reviewing and carrying out planned programmes of care.Assist in
the administration of medication and treatments.Carry out observations in accordance with Trust Policy frequent requirement for concentration.Assisting in the monitoring of patient's nutritional needs.Assist patients, where necessary, with all personal care needs and assist trained staff in assessing patients level of functioning.Escorting patients when directed by the person in charge.Manual handling of patients, where necessary, when indicated in the care plan.Direct contact with bodily fluids.
