An exciting opportunity has arisen for an enthusiastic, dedicated and outcome focused Hospital Liaison Coordinator to join our dynamic team based in Barking and Dagenham. You will undertake duties involved in the day to day operation, management and development of drug and alcohol treatment in Barking and Dagenham and will be responsible for setting up pathways and delivering the full range of services in Queens, King George and Goodmayes Hospital.

The post-holder will also work in Partnership with key partners and the wider integrated treatment team to ensure that these individuals are proactively provided with in-reach services upon admission into Hospital.

You will coordinate regular multi-agency professionals meetings with representatives from relevant Hospital departments, mental health, safeguarding and other involved agencies in order to further identify and implement personalised strategies for supporting particularly vulnerable service users into treatment whilst also reducing the overall rate of hospital admission.

You will provide alcohol / drug harm reduction advice, brief intervention, support Hospital personnel in setting up three way discharge planning, and provide informal training on brief intervention to Hospital personnel.

The role requires flexibility and will include evening and Saturday working.,

  • To develop integrated working within local hospitals which will reduce the impact of repeat hospital admissions for drug and alcohol users as well as reducing mortality rates by setting up robust discharge plans for service users being discharged from hospital into the community.

  • Utilise a range of evidence based assessment tools to assess levels of problematic alcohol use and develop recovery plans based on suitability for community or inpatient detox.

  • To undertake comprehensive assessments for service users, where appropriate and ensure a wide range of interventions are made available.

  • To refer into testing for Hepatitis A, B, C and HIV and provide post-test counselling for results.

  • To develop referral pathways for service users with specialist services.

  • Developing therapeutic alliances with people who have substance misuse problems within a hospital setting and developing effective partnerships with service users to support their individual recovery journeys.

  • Conduct assessments of physical dependency using a symptomatic withdrawal scale, using results to determine safe alcohol detoxification as required.

  • Ensure that procedures are constantly reviewed to promote best practice and evidenced based practice in accordance with UK Guidelines on Clinical Management, NICE guidance, and CGL clinical protocols and procedures.

    Experience of working within substance misuse treatment services.

  • Demonstrable experience of effective partnership working and relationship management.

  • Experience of project development and management.

  • Sound and demonstrable knowledge of harm reduction and health promotion interventions.

  • Knowledge of the issues facing substance misusers including appropriate medical interventions, social care and health issues.

  • Experience of managing a case load and achieving positive outcomes for service users., If you require sponsorship, please note that this role is not eligible for a Health and Care Worker visa.
