Practitioner - West FNC Assessment Team

The post holder will play a key role in supporting the Personal Health Commissioning (PHC) service across the region. Working within the remit of The National Framework for NHS Continuing Healthcare and Funded Nursing Care (FNC), the post holder will deliver an effective and competent level of service and consistently delivering a client-focused service. The post holder will assist and support in the facilitation of patient reviews in line with standard operating policy and procedures and ensuring The National Framework for NHS Continuing Healthcare and Funded Nursing Care (FNC) is implemented and complied with, according to the principles and processes. The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post. Please see attached for full job descriptions.
