Full Job Description

We are looking for a passionate and committed Registered Nurse to join our team at our nursing home in Bognor Regis. The home is dual registered for nursing and residential care for 37 beds but operates at a capacity of 35 beds.

Purpose/mission of the role - To assess and identify patients' needs, then implement and monitor the patient's medical plan and treatment. To work with head of nursing to ensure a safe and effective service whilst enabling service users in positive risk taking and living their life well.

We work in a very collaborative way to offer support to our team by having regular nurse meetings, support from nominated individual (who holds RMN status) and the management team as a whole. We share information with each other learnt from forums and training., · Carry out any wound care as required and ensure appropriate teams have been made aware and that current, up to date information is included within the care plan

· Complete restore 2 forms to monitor vital observations at least monthly as a minimum but more frequently if stated in their care plan

· Monitor nutritional needs including any dysphagia and refer to SALT team as appropriate. Ensure weekly/monthly weights are completed and report any unexplained weight loss to appropriate professional i.e GP/Dietician as identified in the individuals care plan

· Carry out reviews of designated care plans and ensure all information is factual and up to date. Every care plan must be written in a person centred way and should be written/reviewed with input from the service user or their representative.

· Ensure you have seen every service user during your shift to enable you to accurately record and monitor any changes.

· Administer prescribed medication as per care plan. Ensure that medication needs are included in the care plan and are up to date and consider the effects of medication and associated risks. All PRN protocols and medication management forms must be completed upon admission and reviewed monthly.

· Carry out medication audits and report any discrepancies to HON/Registered Manager, ensure sufficient stock of medication is in place.

· Order interim medication and ensure this is signed in / loaded onto the eMAR system in a timely manner.

· Engage in annual pharmacy audits and escalate and actions/recommendations to HON/Manager

· Ensure the medication room is kept clean and tidy

· Ensure all daily notes are recorded in a timely manner. This must ensure everything that happened during that shift i.e. mood, nutritional intake, fluid balance records, any visits, any activities participated in, any health concerns etc.

· Support HON to carry out FNC referrals in a timely manner. Ensure documentation is prepared ahead of these meetings.

· Attend and actively engage in monthly nurse meetings and daily handovers to ensure you are kept up to date of any individuals, home or company changes

· Support HON to complete audits in a timely manner and to accurately reflect so that action plans can be made and monitored, this includes medication, wound care, care plans, resident involvement for example.

· Lead and organise the shift to ensure the best outcomes for service users.

· Complete referrals as appropriate in a timely manner for example to GP's, SALT, physiotherapists, advocacy.

· Actively engage in meetings with other health professionals and take forward any actions required and work with HON or registered manager to achieve these.

· Support the individual and their relative upon admission to enable a smooth transition to the home and begin building positive relationships with those within the home and their support network

· To perform such other duties as may reasonably be required.
