Senior Practitioner

As part of NHS Dorset Personal Health Commissioning Team (PHC), you will manage a caseload of CHC eligible individuals in accordance with National Framework for Continuing Healthcare, Undertake eligibility reviews and assess individuals needs in order to identify most appropriate care options. You will assist NHS Dorset commissioners in searching for care and make recommendations on appropriate placements., You will manage a caseload of CHC eligible individuals in accordance with National Framework for Continuing Healthcare, collaborating with a range of healthcare professionally, community and acute services.

You must be able to work flexibly and autonomously and work collaboratively with Health and Social Care practitioners, patients, families, and carers, ensuring high quality assessments and timely service is delivered. You must demonstrate an openness and honesty and have strong assessment and communication skills and a knowledge of the National Frameworks for Continuing Healthcare, the Mental Capacity Act and Mental Health Act and Care Act 2014.

PHC has 2 offices. East Team is based in Poole, covering Bournemouth, Christchurch, Poole area, and the West Team is based in Dorchester, covering Dorset area. This job advert is recruiting to positions within the East locality and based in Wallisdown, Poole, however you may be required to provide support across both sites as necessary. There will also be a requirement to make off-site visits to undertake assessments and attend meetings and it is therefore essential that you have a full driving licence with access to a vehicle for business use.
