Full Job Description

You will work with education partners in addressing health needs in designated schools and contributing to the Education, Health and Care Plans. You will be responsible for the training and updating of education staff using evidence based training packs, to carry out procedures which include administration of medication and enteral feeds. You will lead on the Initial Health Assessment and drawing up of care plans and work in partnership with school staff, parents and carers in their implementation
Contribute to the public health agenda through Personal Health and Social Education and health promotion activities
Participate in report writing and attendance at Safeguarding Conferences, Reviews and Core Group meetings with support from the Band 6 Special Needs School Nurses, The team provides holistic nursing care for pupils with a wide range of learning disabilities, physical disabilities and neurological conditions as well as implementing the Staffordshire Children's and Young Peoples Health and Well Being Programme.
This role is ideal for someone who is able to work sensitively with children and young people with autism, learning disabilities and complex medical conditions and advocate for their needs.
The successful applicant will work as an outreach nurse and will work mainly in the South of Staffordshire providing support in the special schools covered by the team. The Schools covered by this post are in the following areas Tamworth, Lichfield, Burntwood, Cannock and Wombourne. We also support schools in the North of Staffordshire but the need to travel to this area will be minimal. You will be required to deliver skilled evidence based care to the children within the Special Needs School setting and undertake responsibility for the coordination and orchestration of patient care. This will included being be responsible for the assessment, planning and implementation of care for all clients within the Special Schools with support from the Band 6 Special Needs School Nurses and the Team Leader for Special Needs School Nursing. You will also be supporting Team Leaders in projects and policy and procedure delivery.
Working as a member of a friendly, highly skilled team you will have the opportunity to make a difference to the lives of children and young people who have special needs and their families. You will be responsible for their healthcare needs during the school day and work in partnership with education colleagues in the school and with parents. You will also work collaboratively with the wider Multi Agency Teams to meet health, developmental and safeguarding needs of the children and young people.
The post holder will be expected to work flexibly and will need to be able to transport themselves and any equipment to the different locations, and be expected to work independently around the region of Staffordshire. Travel time will be incorporated in to the working day to support this.

An exciting opportunity has arisen for an experienced and motivated Paediatric or Learning Disability Nurse to join our Staffordshire Special Needs School Nursing Team. We have a 30 hour all year round position available. core working hours will be 8.30- 4.30 over 4 days per week and there are opportunities for some flexible working.
