SPoA District Nurse Team Leader

District Nurse Team Leader will have responsibility and accountability for the call handling and triaging of referrals into District Nursing Single Point of Access (SPoA).
The SPoA District Nurse Team Leader will be required to provide professional virtual nursing care and support to patients, carers and families at the point of referral. Provide support and leadership to all staff. They will be responsible for ensuring a safe service delivery, which will include the development of the staff providing education and training.
The member of staff will be required to function as a member of the multi-disciplinary team, virtually assess individual patient's care needs and subsequently develop, implement and evaluate appropriate plans of care.
The role will also include the supervision and instruction of qualified and unqualified members of the team.
The member of staff would be required to attend training to maintain mandatory skills alongside developing new skills to meet the provision of care required.
