Full Job Description

We are seeking an enthusiastic and dynamic individual to lead an established group of social workers and social care assessors. The team manage varied caseloads which include care and support needs assessments, complex discharge planning, mental capacity assessments and management of risk in a variety of settings.

You will supervise practitioners and support them to promote independence and apply an asset-based approach. You will promote a 'Home First' approach to hospital discharge and opportunities to both reduce and delay the need for formal support.

You will play a key role in ensuring that our people receive the support they need to thrive, while fostering a positive and collaborative work environment across both health and social care.

Devon County Council has a flexible working policy. Currently a combination of office based and home working arrangements are in place, in line with Devon County Council guidance.

The nature of this work requires you to be able to drive and have access to transport. You will need to have the ability to prioritise referrals and manage competing demands simultaneously.

About the Team
The Southern Devon Hospital Discharge Team support people to return to the community following admission to hospital. In line with the Discharge to Assess Model, we support in identifying the most appropriate care pathways to meet people's needs, working closely with Multidisciplinary Teams in complex case management.

The Team manage a varied caseload supporting people who are currently inpatients as well as those who have recently been discharged; undertaking assessments to identify ongoing care and support needs, connecting people to their communities in a strength based manner.

We cover the Southern Devon Footprint; From Dawlish to Dartmouth with Hospital attendance at Torbay Hospital and Dawlish, Totnes and Newton Abbot Community Hospitals.

We support people on rehabilitation pathways, this includes close working with our 10 bedded residential rehabilitation unit, working alongside intermediate care teams and short-term services.

We are an Integrated Service working closely with colleagues in the NHS, Torbay Adult Social Care, Short Term Services Teams, the Voluntary Sector as well as our community-based colleagues.
